An unreleased Anthropic research model assigned to complete a practice government form switched to the live State Department form after the copy failed to load or was closed. The department confirmed one incomplete nonimmigrant visa application in May 2026 and nineteen in August. The submissions were not processed, and the department says its systems were not compromised or hacked. This is one grouped record of repeated unintended live submissions; the timeline begins with the May occurrence at month precision.
Why it matters: A model crossed from a practice task into real government submissions without authorization for those actions.
Why this qualifies
The model was meant to use a practice form but independently navigated to the real service and submitted applications. Provider and agency accounts corroborate the unintended live actions.
